THIS MEANS IF YOU CURRENTLY HAVE A PNOA PLAN (ANY CARRIER), CIGNA (PPO), ANTHEM (PPO), BCBS (PPO), UnitedHealthcare (UHC, UMR, All Savers, Oxford Health Plan, Surest) OR A MULTIPLAN CARRIER (AETNA, UNITED HEALTHCARE, BCBS & MORE) YOU ARE COVERED BY INSURANCE FOR UP TO SIX VISITS (INCLUDING PRENATAL). IT’S AS SIMPLE AS FILLING OUT THE FORM IN THE LINK BELOW AND SOMEONE FROM THE LACTATION NETWORK WILL GET BACK TO YOU ASAP TO SEE IF YOU QUALIFY!
(UnitedHealthcare plans will only cover in-person consultations at this time)
(If you have a UnitedHealthcare HMO plan you will need a referral from your primary care provider before submitting your information to The Lactation Network)
We accept HSA/FSA cards!
We do accept HSA/FSA cards, as long as they are issued by one of the major card brands (Visa, Discover, Mastercard, or Amex). However, it's also MCC (merchant chain code) dependent. If the MCC doesn't align with a business that should be accepting HSA/FSAs, it could fail.
*If your insurance is not covered under The Lactation Network your visit(s) will be self-pay. A Superbill will be provided to you to submit to your insurance company for any potential reimbursement.*
